Oregon Advance Directive 2021
Beginning September 25 Oregon has a new Advance Directive form!
An Advance Directive is a legal form that is helpful in safeguarding your health care wishes. It allows you to name a person to make medical decisions, if you are unable to make those decisions yourself. It also allows you to make decisions ahead of time about end of life care such as pain management and life support. Making these difficult decisions ahead of time can take the burden off your loved ones.
Oregon’s new Advance Directive form goes into effect on September 25, 2021. Here are a few things you should know:
An Advance Directive is recommended for all adults 18 and older, regardless of their health status.
If you have already signed an Advance Directive it will remain valid.
If you moved to Oregon from another state you should fill out a new Advance Directive form. Each state has a different form.
